We are seeking a Community Outreach Specialist to work closely with the Community Outreach Manager to conduct education and outreach within the community.
Primary Responsibilities:
- Network with community based organizations, faith based organizations and other agencies to create partnerships.
- Participates in outreach efforts by scheduling health fairs and meetings as permissible to promote and educate BCFHC programs and services available to the community.
- Assist in the patient recruitment with the HBHS program, schedules and follows up with the potential patient.
- Assist in the development of strategic outreach planning and marketing strategies.
- Assist in the dissemination of educational materials.
- Provide education to eligible patients on the importance of keeping scheduled appointments and seeking regular primary and preventive health care services.
- Participate in training, conference calls, meetings, and committees associated with the program.
- Work with and support other project staff to maintain secure program records and track activities consistent with project protocols.
- Provide PSA support on completing Medicaid applications if permissible.
- Prepare and submit reports timely.
- Attend and participate in local community meetings.
- Attend Safety Committee meetings and assist with efforts to help maintain organization compliance.
Requirements:
- High School Diploma or GED equivalence
- At least one year of community-based outreach experience preferred
- Demonstrated experience working effectively with culturally diversified groups
- Experience working with community residents, congregations, and collaborative initiatives
- Strong communication and interpersonal skills
- Computer knowledge in Microsoft Office and Outlook
- Bilingual preferred (Spanish)
- Ability to work flexible hours
