Community Outreach Specialist

We are seeking a Community Outreach Specialist  to work closely with the Community Outreach Manager to conduct education and outreach within the community.

Primary Responsibilities:

  • Network with community based organizations, faith based organizations and other agencies to create partnerships.
  •   Participates in outreach efforts by scheduling health fairs and meetings as permissible to promote and educate BCFHC programs and services available to the community.
  •  Assist in the patient recruitment with the HBHS program, schedules and follows up with the potential patient.
  •  Assist in the development of strategic outreach planning and marketing strategies.
  •  Assist in the dissemination of educational materials.
  • Provide education to eligible patients on the importance of keeping scheduled appointments and seeking regular primary and preventive health care services.
  • Participate in training, conference calls, meetings, and committees associated with the program.
  • Work with and support other project staff to maintain secure program records and track activities consistent with project protocols.
  • Provide PSA support on completing Medicaid applications if permissible.
  • Prepare and submit reports timely.
  • Attend and participate in local community meetings.
  • Attend Safety Committee meetings and assist with efforts to help maintain organization compliance.


  •  High School Diploma or GED equivalence
  •   At least one year of community-based outreach experience preferred
  •   Demonstrated experience working effectively with culturally diversified groups
  •    Experience working with community residents, congregations, and collaborative initiatives
  •   Strong communication and interpersonal skills
  •  Computer knowledge in Microsoft Office and Outlook
  •   Bilingual preferred (Spanish)
  •  Ability to work flexible hours
